How to Apply in 3 Easy Steps!

  1. Get pre-approved

    If you lived in Baltimore City while working for an approved home care agency or nursing home for at least 6 weeks between March 1, 2020 and August 31, 2022, you can submit your pre-approval application using our secure form. You must be a CNA, CMA, GNA, LPN, HHA or PCA to qualify. It can take up to 4 weeks for your application to be processed. Please be advised that our funding is limited, and distributions will be made on a first-come, first-served basis.

2. Schedule an in-person appointment

Once you are determined eligible, you will receive an email/text to schedule an in-person appointment to provide your proof of employment & proof of residency documentation. YOU MUST come in person to receive your $500 benefit.

3. Pick up your $500!

Come to your scheduled appointment with your proof of employment & proof of residency documentation. These documents MUST be PRINTED COPIES. If you are more than 5 minutes late your appointment will be rescheduled! Once approved, you will collect your digital $500 Visa prepaid card.

You will need to bring a PRINTED COPY of one of the following items on each list to submit your application.

LIST A: Proof of Employment*

from approved Residential Services Agency or approved skilled nursing facilities

YOU WILL NEED TO BRING EITHER:

  • 3-4 Pay stubs approved home care agency or nursing home between March 1, 2020 and August 31, 2022 (including pay period, employer name, beneficiary name)

  • Letter from employer (including employment period, job title, name, address and contact number of signatory)

  • Unemployment Compensation Benefits letter (includes both company and beneficiary name)

  • 2-3 Bank statements (if direct deposit is used and the name of the employer is stated) between March 1, 2020 and August 31, 2022

  • Tax forms are NOT accepted

LIST B: Proof of Residency*

in Baltimore City during employment period

YOU WILL NEED TO BRING ONE OF THE FOLLOWING:

  • Driver’s License

  • Letter/document addressed to beneficiary

  • Government Issued ID card with address

  • Rental/lease agreement with beneficiary listed as a resident

  • Bills showing beneficiary name and address

  • School/medical documents with address listed

  • Court documents with name and address listed

  • Financial institution record with name and address listed (i.e. bank, check cashing, etc.)

  • Voter registration or other government document

* You will be required to bring your photo ID, and a PRINTED COPY of proof of employment and proof of residency to your appointment to verify your eligibility.
